[ACTION CENTER] - Remove Forms and Reports Tab and add Actions Tab

Remove Forms and Reports Tab and add Actions Tab.

Forms and reports will still come but not as separate tabs.

Instead all pending tasks/scheduled tasks will come in a tab.
